A Brooklyn teen fresh out of the city’s tough foster care system is appearing on the Oprah Winfrey Network on Monday night to share what it’s like for a New York kid with no family and no home.

Juliet Forde, 18, hasn’t seen her crack-addicted mother, Nicole Forde, since she was 5, and has bounced around 17 foster homes where she said sexual and physical abuse was the norm.

“I always asked God. Why me? Why are all these bad things happening to me? I want to tell people my story,” said Forde, whose tale airs at 9 p.m. on the show “Searching For … ”

“My purpose in this world is to change foster care,” said Forde, a freshman at Hostos Community College in the Bronx. “God put me through all of this so I can be a voice for all other foster kids.”
